Lincolnshire Dynamic Support Register (DSR)
Who can be placed on the DSR?
In Lincolnshire, we hold an all age Dynamic Support Register which includes children, young people and adults. This helps to support people who might be in transition from children’s services into adult services.
- Individuals need to have a confirmed diagnosis of learning disability and/or autism.
- They must be registered with a Lincolnshire GP.
- They must be at risk of admission into a mental health or specialist hospital.
And/or:
- Children or Young People with a diagnosis of learning disability and/or autism placed in 52 week a year educational setting.
Who manages the DSR?
In Lincolnshire, the DSR is overseen by Lincolnshire Partnership Foundation Trust (LPFT) on behalf of Lincolnshire Integrated Commissioning Board (LICB).
Care and support for people with a learning disability and autistic people who are at risk of going into hospital will be delivered by many different organisations across the Lincolnshire Health and Social Care system, Educational Providers, Community Groups and others.
Why does being on the DSR help?
The benefits of being placed on the DSR include:
- Steering the wider system to identify gaps in current service provision or identify unmet care needs to reduce risk of admission or prevent further escalation of need
- Individuals under 25 years of age at high risk of admission to inpatients services will be able to access the Learning Disability Autism Children and Young People Keyworker Service (LDA CYP Keyworker Service)
- Consideration and access to a Care [Education] Treatment Review (C[E]TR)
- Provide support and promote timely discharge if admitted to an inpatient unit with a specific focus on Learning Disability and/or Autistic needs
How does the DSR identify the level of my needs?
In Lincolnshire, we use a colour system to identify the level of risk of admission for someone placed on the DSR. This helps health and care workers to consider how much risk there is of someone going into hospital because of their current needs.

| Red | For people who are at high risk of going into hospital straight away. |
| Amber |
For people who are at high risk of going into hospital if they do not get the right care and treatment very soon. |
| Green | For people where there is a risk of going to hospital but that risk is being managed well at home with current care |
| Blue | For people who are already in hospital. |

Professionals outside of LPFT can access professional notification forms here. Please note where external professionals (outside of LPFT) are the lead professional for an individual they will retain responsibility for providing updates to the DSR administrative support team in relation to the DSR.
