Request a Prescription

Please complete the following form to order a prescription for your young person.

While we try to fulfill the prescription requests as soon as possible, please allow 14 days for your request to be completed.  Please do not make a request if you have more than 14 days of medication or this may be rejected.

This facility can only be accepted for patients being treated by our ADHD Service.  Once the request has been accepted you will be telephoned to confirm the prescription is ready for collection from Young Minds Matter.

Please Note: due to clinic provision, we do not have the facility for prescriptions to be issued on a Monday.
