Online self-referral

Please be aware

CYP Services include CAMHS (Child and Adolescent Mental Health Service), HML (Healthy Minds Lincolnshire) and MHST (Mental Health Support Teams).  Our service hours are Monday to Friday 9am to 5pm.  The Here4You Access Team screen all referrals each morning, Monday to Friday.  These services do not offer urgent or emergency care.

People aged 16 or over can also access support from adult services. If you would prefer to do so, please visit the Step2Change website to complete a self-referral.

If you believe your life, or the life of another child or young person is at immediate risk, or you require immediate medical attention, please dial 999 straight away or you could go to the nearest Accident & Emergency department.

If it is not life threatening, but you feel you need immediate help you could contact 111 who will be able to advise you about what to do.

Alternatively, you could contact your family doctor (GP), teacher or parent/carer who will be able to help you to seek support.

Support and advice are also available from Childline 0800 1111 or Samaritans 116 123.

Please complete this form in full.  Please ensure any information provided is accurate and reflective of the details your GP holds, if the information provided is inaccurate, we will be unable to process the referral.
