How we partner
Like all NHS organisations, integrated care systems have now been introduced to support health and care organisations in a locality work as closely together as possible. In Lincolnshire this brings together our commissioners, other health providers, local authority, and the voluntary and community sector to deliver as effective and efficient services as possible to meet our population’s needs.
LPFT services are commissioned by two main organisations, NHS Lincolnshire Integrated Care Board who commission the majority of Trust services for the mental health, learning disability and autism contracts we deliver, and Lincolnshire County Council who we deliver children and young people mental health support and mental health social care services on behalf of. Some more specialist services are also commissioned directly by NHS England or our local Police and Crime Commissioner.
Our work has now been integrated through the NHS Lincolnshire Integrated Care System and we have continued our collaborative work with the Lincolnshire system throughout 2023/24 by:
- Leading the system work around our Lincolnshire Mental Health, Dementia, Learning Disability and Autism Alliance, which brings together interested agencies from health, care, police and voluntary, community, faith and social enterprise sector (VCFSE) on the issues that LPFT cannot solve alone and require a multi-agency approach to ensure people of all ages with mental ill health, dementia, a learning disability and/or autistic people, are enabled to live independent, safe, well and fulfilled lives in their local communities.
- Working with our commissioners to support people’s mental health in Lincolnshire by implementing the NHS Long Term Plan for Mental Health and using some of the nationally mandated mental health investment monies to continually improve the support we provide our local community and provide parity of esteem for mental health.
- Continuing our work with system partners to deliver large scale transformation programmes such as the community mental health national transformation programme and newly launched transformation programme for children and young people.
- Working as part of the East Midlands Alliance for Mental Health in our secured setting environments and working together with other providers and commissioners for forensic, Child and Adolescent Mental Health Services (CAMHS) and Adult Eating Disorder inpatient services.
- Taking on the lead provider role for the Midlands Op Courage veterans mental health and wellbeing service, working with seven partners across other NHS trusts, private and charity sector to deliver a Midlands wide provision for veterans and their families.
- Working alongside partners at Lincolnshire Action Trust to deliver our criminal justice liaison and diversion service and new RECONNECT provision for those in contact and leaving criminal justice services.
- Working to develop Lincolnshire’s Dementia Strategy to improve the quality of care and achieve the best possible health and wellbeing outcomes for people living with dementia. The aim is to support people in keeping their independence and resilience for as long as possible.
- Launching a new countywide suicide prevention strategy with partners such as public health and others to acknowledge that suicides are preventable and people’s risk factors are multi-faceted and not only associated with their mental health.
- Being part of a new all age autism strategy and developing our new virtual autism hub which brings together signposting and support information for our autistic community in one place. To help people get support they need prior to or following diagnosis.
Lincolnshire Integrated Care System (ICS)
Lincolnshire’s ICS is a collaborative effort between health and social care providers in the county to improve the co-ordination and quality of care for patients.
The ICS is focused on four key priorities:
- Focus on prevention and early intervention
- Tackling inequalities and equity of service provision to meet population needs
- Delivering transformational change in order to improve health and wellbeing
- Taking collective action on health and wellbeing across our organisation
In common with ICSs across the country, our ICS consists of an Integrated Care Board (ICB), Integrated Care Partnership (ICP) and a range of partner organisations working together.
NHS Lincolnshire Integrated Care Board (ICB)
The ICB have the responsibility for overseeing the delivery of health and social care services across Lincolnshire. It brings together health and social care providers, commissioners, and representatives from the community, to work collaboratively towards improving the quality and integration of services. The ICB’s main priorities are to improve health outcomes for the population, ensure that services are integrated and person-centred, promote financial sustainability, and develop a skilled and sustainable workforce. The ICB is also responsible for implementing the national NHS Long Term Plan at a local level and developing an integrated care system in Lincolnshire.
Joint Forward Plan
With the formation of integrated care boards in 2022 came new requirements for the planning of healthcare services in local areas. NHS England has asked each ICB to develop, and publish, a five-year ‘Joint Forward Plan’ that describes how the ICB and its partners intend to arrange and/or provide NHS services to meet their population’s physical and mental health needs.
Plans include the delivery of universal NHS commitments, address ICSs’ core purposes, and meet legal requirements. Our leaders have worked closely with partners across the ICS to develop Lincolnshire’s Joint Forward Plan, taking account of the Joint Strategic Needs Assessment (JSNA), NHS priorities, and the strategy for Lincolnshire set by the Integrated Care Partnership. Through this process, we have shared information and intelligence on opportunities and risks to ensure our collective plans achieve the triple aim of better health and wellbeing for everyone, better quality of health services for all and sustainable use of NHS resources. Our own plans as an organisation fit with both the Integrated Care Partnership strategy, Joint Forward Plan and Lincolnshire Health and Wellbeing Strategy.
