1.1 Statement on quality from the Chair and the Chief Executive Officer
We are pleased to present our fourteenth annual quality report for the financial year April 2022 to March 2023.
It has continued to be a very challenging period for the Trust and the NHS generally. Our staff and volunteers have continued to demonstrate incredible strength and resilience during the past year.
We have continued to keep the focus on our core purpose, which is to ensure our service users and carers have a positive experience of high-quality evidence-based care. By high quality we mean care which delivers improved outcomes, the best level of recovery possible and results in an enhanced quality of life. To achieve this, we promote strong partnership working alongside effective governance processes. Working in partnership with our service users, carers, staff, governors and other system partners and stakeholders is critical to achieving the highest standards of patient safety, patient experience and clinical effectiveness. We recognise that our services will only ever be as good as the dedicated and skilled workforce we have and that we continue to develop and in which we invest.
This report provides an overview of the quality governance arrangements in place for identifying and monitoring risks and trends to ensure that the Trust works safely and continuously improves. We are proud of our achievements over this last year and passionate about making improvements in the future. Our quality report highlights our successes and strengths, areas we need to improve upon and our quality plans for 2023 to 2024.
We will continue to maintain a strong focus on service user, carer and staff engagement as we move into 2023/24. This involvement will put us in the best place to meet the increasing demand for our services. As a Trust we will continue to work to provide the highest standards of healthcare within Lincolnshire, to support people to live well in their communities.

There are a number of inherent limitations in the preparation of quality accounts which may impact the reliability or accuracy of the data reported.
These include:
- Data are derived from a large number of different systems and processes. Only some of these are subject to external assurance or included in internal audits programme of work each year.
- Data are collected by a large number of teams across the Trust alongside their main responsibilities, which may lead to differences in how policies are applied or interpreted. In many cases, data reported reflects clinical judgement about individual cases, where another clinician might reasonably have classified a case differently.
- National data definitions do not necessarily cover all circumstances, and local interpretations may differ.
- Data collection practices and data definitions are evolving, which may lead to differences over time, both within and between years. The volume of data means that, where changes are made, it is usually not practical to re-analyse historic data.
