Supporting information for referrals

Referrals can only be made with the mother's knowledge and consent, by a GP, obstetrician, LPFT service, health visitor or midwife.

You can access the service by:

Unfortunately, we are unable to accept self-referrals. If you think you need a referral to our service, please seek some support from a health professional who can then refer you. Alternatively, you can self-refer to Lincolnshire Talking Therapies for support.

We are currently accepting referrals for:

  • perinatal trauma (from 8 weeks after birth up to 5 years after birth)
  • baby loss (from 6 months after loss to up to 5 years after loss)
  • pregnancy after loss (no time exclusions)

Please fill all parts of the form as accurately and clearly as possible. You are welcome to contact us if you have any queries about our referral form and/or process.

We ask that when making the referral you gain consent and check that the person is willing, ready, and able to work actively and collaboratively in therapy and able to attend regular appointments.

As per the referral form, please provide sufficient details of the patient’s trauma and loss history and the reason for referral, as this information will help us to determine the appropriateness of our service for meeting the patient’s needs and plan our care accordingly.

Please note that we are not an emergency service, and so cannot provide an urgent response.
