Older adult community mental health teams

Our Older Adult Community Mental Health Teams (OA CMHTs) provide individualised assessment and recovery based interventions and support. This support is for complex mental health related needs and for co morbid emotional disorders and behaviours that challenge in people experiencing dementia. 

We offer you support in your own home and the community and our teams are comprised of:

• Administrative support staff
• Community psychiatric nurses (including non-medical prescribers)
• Occupational therapists
• Consultant psychiatrists
• Associate practitioner support workers
• Access/direct referral to:
  • Older adult psychology
  • Social workers
  • Neighbourhood Teams

Our teams link and work with a wide range of other services to ensure the provision of comprehensive and joined up care. These include social and continuing healthcare providers as well as third sector and voluntary services.

Our older adult community teams are also core members of all countywide primary care based multi agency Neighbourhood Teams (NTs). This enables direct access to seamless multi-agency support for other complex health and social needs such as

• falls
• long-term conditions
• key community support resources

Hours of operation: Monday to Friday 9am to 5pm
