Our Older Adult Community Mental Health Teams (OA CMHTs) provide individualised assessment and recovery-based interventions and support; for both complex mental health related needs, also for co-morbid emotional disorders and behaviours that challenge in people experiencing dementia.

Our teams link and work with a wide range of other services to ensure the provision of comprehensive and joined-up care. These include social and continuing healthcare providers as well as third sector and voluntary services. Further, our older adult community teams are also core members of all countywide primary care based multi-agency Neighbourhood Teams (NTs). This enables direct access to seamless multi-agency support for other complex health and social needs such as frailty, falls, long-term conditions and key community support resources
Our mental health intermediate workers sit within our older adult community mental health teams and align to and support access to mainstream physical rehabilitation services; for older adults with existing complex mental health needs or dementia following or during a period of physical health rehabilitation.
They provide rapid access and time-limited intervention based work in partnership with Lincolnshire Community Health Services (LCHS) rehabilitation services. Within this role they work to support and promote recovery from illness, reduce admissions to hospital/residential care aid facilitate early discharge from acute care.
Our core hours of service delivery are Monday to Friday from 9am to 5pm.
