Community Mental Health Teams

Our community mental health teams provide care for people recovering from an ongoing mental health problem. They support people to live with a mental health condition. You may be referred to this service if you have, or may have a severe or long-term mental illness. We offer you support in your own home and the community. 

Our teams are part of the Triangle of Care and recommend carers to use the Carer’s Trust.

How We Help You

Our Community Mental Health Teams consist of the following skilled staff: 

  • Administrative support staff
  • Community Psychiatric Nurses (CPN)
  • Occupational Therapists
  • Psychiatrists
  • Psychologists
  • Recovery Practitioners
  • Social Workers
  • Support Workers

We offer time limited support using the Care Programme Approach (CPA).  Using the CPA we will help you to develop your own set of planned goals.  We will support you to work towards achieving these.  

Discharge is an integral part of the recovery process.  Where appropriate we provide a Fast Track programme, to support your continued recovery.

All our Community Mental Health Teams work closely with other local mental health services.  Such as the Crisis Resolution and Home Treatment teams to prevent relapses.  Our Teams work with the Lincolnshire's Managed Care Network of voluntary and community groups to give people support, structure and choice in their lives.  

We will also work closely with your referrer whilst receiving our services.  We have a dedicated lesbian, gay, bisexual and transgender (LGBT+) lead.  

Many of the Community Mental Health Teams offer group activities such as:

  • Peer Support Groups
  • Recovery College
  • Sport Groups such as Tennis, Golf, Swimming
  • Social and activities Groups such as Ten-pin Bowling, Arts, Crafts, Allotment
  • Exercise and Education such as Emotional First Aid, Employment Advice, Walking Groups
  • Carers group

What Services We Offer & Advocacy

We offer the following services and interventions to support each person's medical, social and personal recovery:  

  • Cognitive Behavioural Therapy.  Both clinical and family based work, either individually and/or in therapy groups
  • Risk Assessment and Management
  • Medication Initiation and Management
  • Physical Health and Wellbeing support
  • Relapse Prevention
  • Wellness Recovery Action Planning (WRAP)
  • Support to access personal budgets, employment and education 
  • Support to take part in social activities and training
  • Support to your carers and education for your carers

Advocacy

Hours of Operation & Referral & Teams

Our core hours are Monday to Friday from 9am to 5pm.  If you need support outside of these times, our staff do work flexibly within the community to meet individual needs.

Referrals

To access the service you will need to be referred by your GP, or other Mental Health care service.  All referrals are managed through Single Point of Access (SPA) Telephone:  0303 123 4000 (charged at local rate)
