Community Rehabilitation Team

If you are experiencing difficult times and trying to get back on your feet, you may have lost your confidence and be finding things more challenging. The Community Rehabilitation Team work with you to empower you to re-build your confidence, by promoting and developing skills and listening to the areas which are important to you, building hope for your future.

Areas the team can help include:

  • Finding practices and strategies to help reduce stress and manage challenging situations

  • Helping you to feel confident, in control and independent in your home

  • Supporting you to find and be part of activities, groups and events in your community

  • Helping you to improve and maintain relationships with those  around you, such as family members, carers or other support networks

The Community Rehabilitation Team is made up of people who can work alongside you and your Community Mental Health Team to support you with activities of daily living, re-build your routine and access in the community.

Who is in the team?

The Community Rehabilitation Team is made up of a variety of professionals with different skills, some of these include:

  • Peer Support Workers
  • Community Psychiatric Nurses (CPN)
  • Occupational Therapists (OT)
  • Social Workers (S/W)
  • Community Support Workers (CSW)
  • clinical Psychologist
  • Consultant Psychiatrist
  • Housing Lead

The team will support you to plan and help you to achieve meaningful goals. You will always be central and fully involved in planning the work the team do. The team will work together with you to create a clear plan that will highlight the members of the team you will be working with moving forward.

Your support from the Community Rehabilitation Team will be tailored to your own goals and this will be reviewed regularly and can be adapted.

How to contact the team

The Community Rehabilitation Team works 8am - 8pm, 7 days per week.
