Integrated Place Based Team (IPBT)

The Integrated Place Based Team (IPBT) is a mental health service for adults and older adults that offers support in the community. The team help people with Severe Mental Illness and people who are struggling with their mental health and/or have complex needs. IPBT work with service users to help them build the resilience and strategies to enable them to manage their own mental health.

The service has been designed to offer support to those that do not meet the criteria for other community services, who still require support for their mental health.

There are twelve innovative IPBTs in Lincolnshire which work collaboratively in the Primary Care Network (PCN).

The team work closely with Primary, Secondary, Healthcare and Social Care services with a holistic approach. They work closely with the other mental health services and can support people to move between services if/when needed.

The team includes a variety of mental health care professionals, including; Mental Health Practitioners, Peer Support Workers, Social Prescribing Link Workers, and Admin Support Staff.

We use a Trauma Informed Care (TIC) model because we recognise the importance of understanding the impact of people's narratives so that we can support them with personalised care planning.

Aims of the service

  • Enable people to manage their condition or move towards individualised recovery on their own terms. The aim is for them to be supported in their local community by their families, carers and social networks.
  • Improve access to mental health support in the community by bridging the gap between voluntary, health and social care. The team develop relationships with community partners to inform a range of holistic services.
  • Offer a person-centred approach to promote health and wellbeing through collaborative care planning and recovery. 
  • Ensure the needs of carers are met and that they are involved in planning.
  • Provide assessment, brief evidence-based intervention and signposting to enable service users to live well within their communities. 
  • Work with service users using psychologically informed interventions.
  • Improve education and monitoring of physical health and medication.
