The Lincolnshire RECONNECT team are celebrating the service’s first anniversary.
The team is comprised of Lincolnshire Partnership NHS Foundation Trust (LPFT) and Lincolnshire Action Trust (LAT) staff working in partnership to improve the continuity of care of people leaving prison and tackle health inequalities of people within the criminal justice system.
Lincolnshire RECONNECT was first launched on 3 July 2023 at HMP Lincoln, followed by HMP North Sea Camp in October 2023. The final launch will be at HMP Morton Hall later in July 2024.
The team are proud to have received an LPFT staff recognition award for continued hard work and outstanding contribution over the past year and are looking forward to working together over the next year.
