Individuals are understood holistically and supported in their own communities through a multitude of connected assets including primary and secondary care, local authority, voluntary and social enterprise sectors.

Senior Mental Health Practitioner vacancy

This role would involve clinically leading the new Integrated Placed Based Teams in the Spalding area. This is an opportunity to work as a valued member across primary and secondary care and with the neighbourhood teams to locally support the mental health transformation. We are looking to recruit core professions with a wide breath of mental health knowledge and experience.

In addition to leading the team, you would undertake triage, mental health assessments and take on a caseload of highly complex service users to provide therapeutic interventions and co-ordinate the patient’s journey across the health, social care, and voluntary sector system, regardless of care setting.

For this post we would be looking for someone an experienced clinician with proven skills in working with people who have a range of highly complex mental health needs, demonstrating practice that is evidence-based and innovative. You would demonstrate adaptability, excellent communication skills and self-motivation.

 

Mental Health Practitioner/Older Adult Mental Health Practitioner

This is an exciting role in the new Integrated Placed Based Teams in the Spalding area. This is an opportunity to work as a valued member across primary and secondary care and with the neighbourhood teams to locally support the mental health transformation. We are looking to recruit core professions with a wide breath of mental health knowledge and experience.

This role would include triage, initial assessment and management and prioritisation of a caseload dependent on the individual’s health status. The successful candidate would deliver interventions that are person centred.

For this post we would be looking for an experienced clinician with proven skills in working with people who have a range of highly complex mental health needs, demonstrating practice that is evidence-based and innovative. You would demonstrate adaptability, excellent communication skills and self-motivation.
