An introduction to the Memory Support Service

The Dementia Support Service, an LPFT service which is commissioned by Lincolnshire County Council (LCC), changed its name to the Memory Support Service in December 2023. This is to reflect the fact the service also supports people with mild cognitive impairment (MCI) and not just those living with dementia.

The Memory Support Service works alongside community mental health teams, memory clinics and partner organisations across the county. The service gives service users access to a Support Advisor who will provide guidance and advice, including signposting to partner organisations that can provide a range of further support such as financial support, advice for carers, and local activity groups.

This is a non-clinical service and in most cases, this guidance will be provided via phone or online. However, in some circumstances, the team will be able to visit service users in person.

The service supports people in Lincolnshire with:

  • understanding the diagnosis and what this could mean for them and their carer or loved ones.
  • understanding the importance of planning for the future.
  • accessing the right support to enable them to live in the local community for as long as possible. 
  • building resilience and confidence to manage the impact of the diagnosis.
  • identifying the range of support services available.

 

Referrals to the service

Referrals to the service can be made by members of the public, as well as professionals in health and social care, such as GPs and members of staff at LPFT.
