As a Trust we regularly review our processes to make sure we are providing the best care possible to people who access our services.
To help us continually improve, our quality improvement team is currently looking at mental health care plans and how we can improve our process of developing these with you and your carers and/or family.
To help us with this we want to hear from people who are using, or who have recently used our services, and/or their carers, and families, so we can understand what is good, what could be better and what might be missing from mental health care plans.
We have developed this short survey to gather people's views, and we'll be talking to people over the next few months as we look at how we might want to change care planning in the future.
