Community Mental Health Survey 2021

LPFT will be carrying out a survey between February and June to find out what patients think about the care they receive. Feedback from patients is vital and tells us their views and priorities for bringing about improvements in the quality of care. The results help us see where we perform well and to identify any areas where there is room for improvement.

The survey is run every year using the same method and asking the same (or similar) questions so that we can see any changes in patients’ experiences over time. This enables us to make improvements in a specific area if people’s responses show a poor experience. It also helps us to evaluate the success of quality improvement initiatives implemented since the last survey.

The questionnaire will be sent to a sample of patients who used our services. Patients are being asked about various aspects of their care, including the quality of care and treatment, communication with staff and information provided.

This is a part of national programme coordinated by the Care Quality Commission (CQC) to improve quality of care and patient’s experience. Taking part in this survey is voluntary and all answers are confidential.  If you are selected to take part, your contact details will be used by researchers to carry out the survey.
